Nurse: Good morning. I wanted to go over Mr. Johnson's care plan for this week.
Caregiver: Of course. I have my notebook ready.
Nurse: His blood pressure medication is taken twice a day — eight in the morning and eight in the evening, always with food. The water pill is only in the morning.
Caregiver: Got it. What should I watch out for?
Nurse: If his blood pressure reads above 160 over 100, call me immediately. Also watch for swelling in his legs or feet — that can be a sign of fluid retention. He should walk for at least ten minutes twice a day if he feels able.
Caregiver: And if he refuses to take his medication?
Nurse: Note it down with the time, and call the family. Never force him. Document everything — we need accurate records.

Receptionist: Hello Anna. What seems to be the problem?
Caregiver: He's been complaining of chest pain since this morning. It started around nine AM — a dull ache on the left side. He also seems more short of breath than usual.
Receptionist: Has he taken his heart medication today?
Caregiver: Yes, I gave it to him at eight o'clock with breakfast as usual. But the pain hasn't gone away.
Receptionist: I'm going to put you through to the on-call doctor right away. Please keep him calm and seated. If the pain gets worse, call 999 or 112 immediately.
